How do I schedule my preventive care appointment?

Call our office and setup an appointment for all current members.

Can I add members to my account?

Absolutely! You can add any dependent children to your membership.

Can I change or update my billing information?

Yes! Call our office to make any changes to your account.

If I cancel, can I get a refund on my membership?

No. Refunds will not be provided for dues paid under any circumstances, including failure to schedule and maintain appointments.

How do I cancel my membership?

Please call our office to make any changes to your account. All changes will take 15 days to go into effect.
